We provide same‑day certified copies of birth and death certificates in our offices. You can apply during regular business hours, and we print the certificates while you wait.

Birth Certificates

If the person was born in Missouri, they or their immediate family members can request a certified birth certificate. Immediate family members for birth certificates include: spouse, parent, sibling, grandparent, child, aunt, and uncle. (Cousins and “great” relatives are not included.)

You will fill out a short application. The cost is $15 per copy. To apply, please bring:

  • The person’s full name
  • Date of birth
  • A valid driver’s license for the person making the request

Death Certificates

We can provide certified death certificates if:
  • The death occurred in Missouri, and
  • The death was in 1980 or later
 
To apply, you must be a family member of the person who died. Bring the person’s name and date of death. You will complete an application in the office. The cost is $14 for the first copy and $11 for each additional copy.
 
If you are not a direct family member, please call our office. We can tell you what extra documents you will need. Family members for death certificates include: any family member, including in‑laws, cousins, “great” relatives, and step‑family.

Acceptable Identification Documents

Primary Documents (1 needed)

  • A state-issued driver’s license that includes a photograph and date of birth  
  • A state-issued identification card that includes a photograph and date of birth
  • A U.S. military identification card that includes a photograph 
  • A U.S. passport with a current photograph 
  • A school identification card/document showing the applicant’s name, photograph, and date of the school year 
  • Work identification card that includes the applicant’s name, photograph, and company name 

Alternate Forms (2 needed)

  • Letter from government or social agencies  
  • School yearbook 
  • Signed Social Security card or Social Security printout (printout of an applicant’s    Social Security account of activities)  
  • Court-certified adoption papers that include the adopted parent(s) name  
  • Official certified deeds or title to property 
  • Certificate of vehicle title or registration documents 
  • Proof of auto insurance 
  • Insurance policy (health, home, life, etc.) 
  • Medicaid/Medicare document or identification card 
  • A payroll stub that includes a Social Security number of the applicant (cannot be handwritten stubs) 
  • Military discharge document (DD-214) 
  • Canceled duplicate check (must show name, address, signature, and name of institution) 
  • Utility bills that show the name and address of the applicant (water, gas, electric, telephone) 
  • Shelter name band (including name of shelter)
